voxels in dicom slices do not need to be aligned to each other. So it might be like ++++++++++++++++++++++++++++++++++ ++++++++++++++++++++++++++++++++++ ++++++++++++++++++++++++++++++++++ where they are shifted relative to each other. That is something ft_read_mri cannot deal with yet. Technically I think it would be possible to represent this in mri.anatomy and mri.transform, where the 4x4 would represent the "shear".

without access to data that demonstrates the skewed/sheared voxel alignment, I cannot resolve this.

This closes a whole series of bugs that have been resolved (either FIXED/WONTFIX/INVALID) for quite some time. If you disagree, please file a new issue on https://github.com/fieldtrip/fieldtrip/issues.
